SwitchForms is a web-based drag-and-drop form and survey builder for agencies, product teams, and operators. Publish a hosted form or embed it on your website, then connect structured responses to your application or team.

Start building

Build and publish

Start with a template, reorder fields, and preview the respondent experience on mobile. Configure validation, branding, approved embed domains, and hosted links. Starter and higher plans add conditional logic, calculations, answer piping, and custom CSS.

Conversational presentation can show one question at a time. Optional same-device recovery keeps non-file answers for up to seven days; it does not recover uploaded files or create cross-device resume links.

Run finite surveys

Growth and Scale support NPS, CSAT, CES, ranking, matrix, and allocation questions with version-aware reporting. Surveys close after 14 days by default, with configurable windows up to 90 days. Scale adds advanced research controls.

Create a new survey for each recurring wave. Editing or republishing does not restart its collection window.

Connect your application

Published forms provide stable field IDs, a field/value map, JSON Schema, cURL, and TypeScript examples. Growth and Scale can accept version-validated JSON through scoped API keys.

Historical responses retain their submitted version. Private uploads remain in the authenticated portal; notification emails do not attach them.

Build with your AI assistant

Paid workspaces support opt-in Model Context Protocol (MCP) connections. Use a Streamable HTTP client with API-key authentication to create and refine form drafts. Survey drafts require Growth or Scale.

Enable AI access, grant scoped keys, and review and publish in SwitchForms. Bring your own AI assistant and model subscription. Normal workspace permissions and plan limits still apply.

Route the next step

Scale supports response routing, owners, due dates, escalation, and connected one-time Stripe Checkout payments. Payments use a server-verified calculated total and Stripe-hosted onboarding.

Choose a plan

Free supports basic forms. Starter adds logic and customization. Growth adds surveys, private uploads, and APIs. Scale adds workflows and connected payments. The first workspace includes a 14-day Scale trial without a card; additional workspaces start on Free with their own usage and plan.

At the monthly response limit, new submissions pause until reset or upgrade; existing responses stay available. See current prices and exact plan limits.
